                  I.    The Medical Center at Elizabeth Place is a "physician owned hospital". If you would like to                         enquire about these physicians, please call: 937-223-mcep (6237) or
                        e-mail ask@mcep.us.

                  II.   You have a right to choose the provider of your health care services. Although we know the                         Medical Center at Elizabeth Place is able to meet your needs, you have the option and                         use another facility other than the Medical Center at Elizabeth Place. You will not be                         treated differently by your physician if you choose to use a different facility; however be                         aware that he/she might not be able to perform your procedure at another facility if                         he/she does not have privileges at such facility.
